Joining the Transverse Myelitis Society is easy. As a member you’ll receive a Welcome email with lots of helpful information to guide and support you. You’ll also stay informed with regular email updates about the latest news, events and opportunities to connect with other TMers in UK and beyond.
Membership is completely free for people with TM, ADEM, NMO,NMO-SD,AFM,MOG-AD and related rare neuroimmune conditions.

How to use this form...
All individuals applying for membership must complete fields numbered 1 to 4. Any field marked with an asterisk (*) is required, and the form cannot be submitted until these fields are completed.
If you are registering yourself, complete field 4 and then continue to field 8 (Date of Birth) and onwards. If you are registering on behalf of someone else, select “No” in field 4 (“Are you registering yourself?”), choose your relationship to the person with Transverse Myelitis, and complete all remaining fields using their details.
When all required fields have been filled in, click “Send” to submit your application. A confirmation message will appear to confirm that your application has been successfully submitted.
